Milestone Home Health Care 

MSHHC's Complex Care Management program supports people living with chronic illnesses and their choices on how to manage their health. We create a supportive service care plan that is unique to the person's condition and needs. This person-centered process focuses on what's most important to the person and their meaning to "quality of life".

A typical supportive service care plan may include:

  • Condition specific assessment/recognition of signs and symptoms
  • Vital Sign Monitoring
  • MD appointment and medication reminder
  • Support prescribed activity level and diet
  • Caregiver Support
  • Fall Prevention/Assessment
  • Home Safety Evaluation
  • Personal Care
  • RN Supervision

All training and education is provided by a Registered Nurse and is in accordance with the National Standards and Recommendations of the American Heart Association, American Diabetes Association, American Lung Association, and the Center of Disease Control (CDC).

Some health conditions require specialized care management support to prevent unnecessary hospitalizations and disease exacerbation. Complex Care Management is MSHHC's chronic illness training support program that our caregivers undergo in order to provide disease and condition specific supportive care.
